Small area requires a new slab (1.2m x 1.5m).

I have dug out enough for;

100mm hard-core,
25mm sand,
Membrane
100mm concrete,
40mm insulation,
50mm flow screed.

The amount of insulation was dictated by other floor levels.

I have read conflicting information regarding the ballast to cement mix. Some say 5:1, some say 3:1. Any recommendation from here?

5 & 1 is good: 3 & 1 is too strong for normal concreting applications. All measures by volume.
