I am in the process of creating Project Sheets that I eventually will get printed. They are cvo/extras/completion sheets, I have always had a duplicate book that I have used for this kind of stuff, but I want to have a more professional looking thing, with the logo at the top blah blah. Does anyone have a template or a layout they have found works well? ....any input appreciated

It's initially to type on a computer, I want to have them to use just now and see if they work and then I will get it printed up as a duplicate book. We have been scribbling on a duplicate pad for extras, cvos etc for an age and would really like a nice smart logoed sheet.

I have just designed some letter heads with microsoft publisher. I'ts pretty straight forward and you can upload any image you want and mess about with the positioning. Plus you can add text etc etc I'll see if I can get a sample on here in a bit.

I have letterheads and all the various software - although I use Excel mainly for this type of thing -what I wanted was to get the wording to get a single document that covered all on-site requirements as I said - it is more others' experience of what they get signed off...or record on a day by day basis. Maybe I am just paranoid but I record everything and get everything signed off. I have included the project description then allowed an area for a cvo - so any change, addition or subtraction, extra materials supply only, extra hours and an area for the job to be signed off. There are like standard forms for this and I wondered if anyone used a template or had also custom made one themselves that worked particularly well.

Have you had a look online? There are daywork sheets and stuff like that floating around so you may find something to suit or fine tune.
