perhaps a mixer tap is passing cold mains into the hot pipe, and up to cold water tank

is the water warm in tank?

you need to isolate the shower, washing machine, and any other mixers to find out which is faulty, rather than hiding the problem with a non return valve

yep, it could well be.

i would think the shower is tank fed, rather than from the mains from the cold.

you could try running the hot in kitchen mixer, until pipe is hot, then feel the pipe, once you turn it off, see how quick it goes cold.

if its only a few seconds, then that'll show the cold mains is going into the hot pipe
